Terms and Conditions

Payment is required to confirm the booking. Price includes postage paid return. RETURN LABEL IS INSIDE BOX! PLEASE SAVE YOUR BOX FOR RETURN.

Rental time commences once you receive your unit. Please contact Jillian Silva at 512-773-2375 with any questions.

  1. The ELLE TENS should only be used in accordance with the instructions provided and only by the customer name that is renting the unit.
  2. The success of the ELLE TENS in labor is neither guaranteed nor warranted by Jillian Silva.
  3. The ELLE TENS must not be used by anyone fitted with a cardiac pacemaker, or suffers from epilepsy. If you have any concern whatsoever consult your medical advisor prior to use. The
    ELLE TENS unit must not be used in water. (Bath or shower) Please turn off unit and remove pads before getting in water. Unit may be reconnected once your are out of water and your
    back is dried off well, so that pads adhere well.
  4. The ELLE TENS must not be used until the pregnancy has reached full term (37 weeks).
  5. Failure to return the ELLE TENS at the expiration of this 5 week period will result in the Customer’s credit card being charged a fee of $175 unless previous arrangements have been
    made. If the unit is not returned for any reason the Customer’s credit card will be charged a $175 fee.
  6. It is possible to extend your rental period with notice to Austin Childbirth BEFORE the TENS unit is due back AND if the unit has not been reserved already for another client. The fee to
    extend the borrowing period is $10/per week.
  7. PLEASE NOTE: A $5 late fee will be charged for each additional day that the TENS unit and accessories are late. I agree to additional charges to be automatically placed on my credit
    card.
  8. Non-use of the rented ELLE TENS is entirely at the discretion of the customer and no refund will apply.
  9. The Customer should check the contents of the rental package upon receipt and familiarize herself with the ELLE TENS and the instructions. The Customer must advise Jillian Silva of any problems whatsoever immediately. Jillian Silva accepts no responsibility where the Customer fails to do so and renders it impossible for Jillian Silva to take any corrective action.
  10. In checking the terms and conditions box above and submitting the send button below, the Customer agrees to all the terms and conditions.
